We are seeking passionate and talented individuals aligned to our values to help us deliver on our purpose.How You Will Make a DifferenceClient SupportSupervise and deliver quality support workers to our Participants.Provide coaching and guidance to Support Workers on complex clients.Contribute to the development, implementation, and review of Participant support plans.Assist in development of weekly program of skills enhancement and activities designed to meet goals identified in Participant support plans.Schedule and assist Participants to attend medical and allied health appointments and oversee practices which uphold the health and well-being of the client(s).Monitor adherence of the support team towards Participant support or recovery plan(s) and weekly program of support activities.Identify and assist in locating resources, services, and supports that facilitate community integration including social, educational, vocational and leisure activities.Assist with Participant case reviews (both internal and external to Open Minds) to ensure client service continuity.Team SupportAssist in the induction, orientation and mentoring of newly appointed Support Workers.Participate in the on-call roster rotation to support staff outside of normal working hours.Manage and maintain accurate client information and file notes and complete administrative and documentation tasks in a timely and accurate manner.Prepare a comprehensive monthly performance report for the Care Leader and provide information to facilitate the completion of internal and external reports as required.Your Qualifications and Competencies For This RoleFormal Qualifications (at Certificate IV level or higher) in Mental Health, Disability, Community Services, Social Welfare or a related discipline are highly desirable.Current Blue and Yellow Card.Current driver’s licence.Operational and applied experience supporting people with a mental disability living in a community residential setting.Demonstrated skills to develop individual support plans and risk management plansDemonstrated skills in coaching and mentoring less experienced staff.Ability to work independently, collaboratively and apply business judgement.Strong communication and interpersonal skills.Understanding of the Disability Services Act (QLD 2006), Mental Health Act (2016) and Human Services Framework is desired.Willingness to support complex clients with a range of mental and physical health issues.Benefits For youOn top of joining a supportive and positive workplace culture, Open Minds will give the following back to you as an incentive for your excellent support provided to our Support Workers and Participants.Permanent Position – Full-Time.Hourly rate at SCHADS Award Level 4.Option to Salary Package up to $15,900 of tax-free income.Extra 20% Leave Loading on top of your annual leave pay.Access to our free counselling service.In-house training and professional development opportunities.Job security in our growing business. 60000 AUD Townsville
