Applications are now open for an innovative and experienced Integrated Mental Health Officer to join the team at Head to Health Lismore.As one of our Integrated Mental Health Officers you will deliver psychosocial interventions using a motivational interviewing framework. This includes education, skills development, self- reflection, improving self-care, increasing community and social connection and engagement in structured activities.You will manage the overall implementation of the client’s change plan and ensure that there is timely and effective coordination between internal and external stakeholders supporting the client including the person’s family or significant others. Benefits include:Opportunity to salary package up to $15,900 of tax-free incomeProviding team-based provision of service for people under a flexible practice modelWorking in a vibrant centre with other staff who are passionate about working with people from their community.Individual supervision, and peer reflection groups.Professional development and training opportunities provided.Reception and administrative support.Consulting rooms, activity space, and resources On a day-to-day basis you willCollaboratively develop change plans for people to assist with addressing their mental health and psychosocial needs, as well as having primary responsibility for management of the plans.Provide client-centred care to people, while incorporating family / significant other inclusive practices where appropriate.Conduct risk assessments and develop thorough and practical risk management plans.Maintain up-to-date, high-quality, contemporaneous client records and comply with data collection standards.Comply with continuous clinical quality improvement activities to improve the service provision and outcomes.Attend case review meetings, and participate in individual supervision, peer reflection and team meetings.Engage in activities to continually develop the Head to Hub model of practice including but not limited to developing processes, documentation, and new initiatives Does this sound like you?Tertiary qualifications in health, behavioural/social sciences, community services, or a related discipline and/or extensive experience in the mental health sector Experience providing support coordination and psychosocial support in a mental health setting.Experience in developing, managing and implementing change plans for clients with mental health difficulties.
